Application for a national health insurance card-EHIC

From 1 July 2018, health insurance companies no longer issue new health insurance cards to holders of ID cards with an electronic chip. They are only issued with the European Health Insurance Card (EHIC) by their health insurance company. The ID card or the EHIC is used to prove the identity of these insured persons to a physician or a pharmacist. The changes only apply to applications for new cards, the previously issued cards can be used until their expiry date.
